Welcome to the North Carolina Advance Health Care Directive Registry! 

Three (3) Advance Health Care Directives forms and an Organ Donor Card form are located here for your information and use. The Organ Donor Card requires two witness signatures. You may complete the forms online and print the completed forms for notarization. If you prefer, you may open the blank forms with Adobe Acrobat ™, print the blank forms, and complete them later. The documents will be made available online when submitted to the Secretary of State's Office with a $10 fee per document.

Whether or not your Advance Health Care Directives are registered online with the North Carolina Secretary of State, please be sure to tell your family, your medical providers, and your attorney about your directives and your wishes.

Documents available in the registry are only accessible over the Internet. To access these documents, a person must enter a unique file number and password.

Documents, file numbers, passwords and other information in the Advance Health Care Directive Registry are confidential and not subject to public disclosure. North Carolina law requires the Department to protect this information from unauthorized disclosure.
